Grampians Regional Palliative Care Team (acute)

The GRPCT aims to support General Practitioners in the delivery of highly complex palliative care in the acute sector, aged care residential facilities, in the home or community. The team includes palliative care physicians and a registrar, a nurse practitioner and clinical nurse consultant. A written referral utilising the SCTT referral tools is preferred and all referrals are triaged by the team.

The GRPCT provides secondary consultation (see contact details above). All community referrals into GRPCT are discussed with the patient and the treating general practitioner prior to the service seeing the patient.

The GRPCT provides education, information and resources to General Practitioners. Please contact Jade Odgers, Manager on 5320 3535 to discuss education requirements.

The Grampians Regional Palliative Care Team aims to facilitate the ongoing development of palliative care services in the Grampians region so that people throughout the region have equitable access.

  • Sporadic exacerbation of symptoms requiring emergency treatment or immediate change in the care plan

  1. Progressive incurable disease
  2. Complex symptom control problems
  3. Patient agrees to referral
  4. Referral based on patient/family need, NOT on prognosis
